You do not need a debit or credit card to apply online. You can submit an
online application form and then send us a cheque or a postal order.

Apply for a Child Deed Poll
For changing the name of a young person who is under 16 years of age
(the applicant must have parental responsibility for the child).
Apply for a Family Deed Poll
For changing the surname of a family (at least one parent and a child who is under 16 years of age). Note: the parent(s) can also
change their forenames if required
(if any son/daughter is 16 years of age or over, you
will also need to submit a separate adult application).
