    Section 4
Do you require our Legal Copy and Archive Service?
Organisations such as the Passport Office, DVLA, the financial institutions etc will not accept a photocopy of your Deed Poll document. Our legal copies (also known as certified copies) will be accepted by these organisations in lieu of your original Deed Poll document thus avoiding your original Deed Poll being lost or damaged in the post. Also, we archive an image of your signed Deed Poll so you can order further legal copies in the future should you ever lose or mislay your Deed Poll.

The fee for our Legal Copy and Archive Service is £13 plus £1 for each legal copy ordered (two copies minimum). For further information about our Legal Copy and Archive Service and how the service works click on this link (opens new tab).

    Section 5
Are you making this application because you need to replace a lost Deed Poll or because you need a Deed Poll as evidence of a past name change?
If this application is to provide you with a replacement for a lost Deed Poll (or a lost Statutory Declaration), or you are applying because you need a Deed Poll as documentary evidence of a past name change (for example, your name was changed by usage when you were a child), we can modify your Deed Poll to include a declaration that states you have been using your current name (i.e. the new name you have entered in section 1 above) for all purposes since a particular date. This declaration explains to those you show your Deed Poll to that it is not for a new name change.
If you would like this modification made to your Deed Poll, for which a £4 fee is payable, please select the option Yes, the details are as follows and then answer the questions to tell us when and how your name was originally changed to your current name. If you are unsure whether you need this modification made, please call our Helpline for advice or email us.

If NO then proceed to section 6

If you require this modification, in section 1 above, please ensure you have entered your former name in the current name boxes and your current name in the new name boxes. Also, the address you enter in section 1 above should be your current address and not where you were living when you previously changed your name.

    Section 6
Do you wish to claim a fee reduction because you have changed your name with us before?
You can claim a £10 fee reduction on the total value of your order if you have changed your name with us before i.e. if you have previously purchased a Deed Poll direct from us.

Please note, this reduction cannot be claimed in addition to the fee reduction for receiving a qualifying benefit.

If NO then proceed to section 7

If claiming this reduction, go to section 8 after completing this section.

    Section 7
Do you wish to claim a fee reduction because you receive a qualifying benefit?
You can claim a £10 fee reduction on the total value of your order if you receive either:

    • Income Support
    • Income related Employment & Support Allowance
    • Pension Credit
    • Income-based Jobseeker's Allowance

    • Universal Credit (if your household income is less than £20,000 per annum)

The fee reduction is only available if you send us written evidence of your benefit entitlement (an original award notice, not fax or photocopy, dated within the past 3 months). If you need to obtain a new award notice click on this link (opens new tab) for contact details. Please post your award notice to our address shown in section 10 below.

Please note, this reduction cannot be claimed if you are claiming the fee reduction as a previous client.
