Acknowledgement by the person recorded as the father on a birth certificate that he is not the biological father
The following letter is an example of what we require from the person recorded on a child’s birth certificate as the father (i.e. the legal father) acknowledging he is not the biological father.
If more than one child’s name is being changed, please provide a separate letter for each child.
