Family Deed Poll Application Form

To change your family’s surname (family means at least one parent and one child under the age of 16) please complete and submit the secure application form below, which has provision for 6 children.  Please note, the parent and their spouse/partner can also change their title, first and middle names if required but children can only change their surname to the new family surname.

You do not need to provide debit or credit card details to apply online – you can ask us to call you for your card details or you can pay by cheque or postal order.  Before you complete this form, you may wish to read about our terms and conditions, fees and your statutory rights including your right to cancel by click on this link (opens new window).

After you have submitted your application, you need to send us a letter of consent (by email, fax or post).  A link to a page containing example letters of consent will be provided after you have submitted your application.

You must be the mother or father (with parental responsibility) for the child(ren) whose name(s) are being changed to complete this application form.  An explanation of parental responsibility is given on our page Can I change my child’s name (opens new window).  This article will also allow you to determine whether you have sole or joint parental responsibility for the child(ren).  If you do not have parental responsibility, please call our Helpline for advice about how to proceed.

    Section 1
    Applicant's details (can be the mother or father)
    Preferably, you should be the mother but you can be the father if you have parental responsibility for your child(ren). Your current name, new name and address will appear on your Deed Poll document. Your title will only appear if you are changing it.

    If you select no we will issue your Deed Poll un-dated for you to date when you sign it. If you enter Yes, we will date your Deed Poll for 2 working days after we post it (when you should receive it by) or you can enter s specific future date.     Details of your spouse/partner (if also changing their name)
    A separate Adult deed poll will be issued to your spouse/partner.

    Section 3
    Children to be included on your Deed Poll
    Your application MUST include at least one child under the age of 16 for which you have parental responsibility. Your children can only change their surname to the new family surname. If you wish to change a son/daughter's first or middle names, you must also apply for a separate Child Deed Poll (if under 16 years of age). If 16 or over, you must apply for a separate Adult Deed Poll after you have submitted this application.

    Which service do you require and where do you want your Deed Poll documentation posted to?
    The fees shown are per order. For example, if you order two Deed Polls using our UK Priority service, the total service fee will be £4 and not £4 for each Deed Poll. Working days are Monday to Friday excluding bank holidays.

    UK Priority: +£4

    (if application received by 3.30pm Monday to Friday).
    UK Express: +£10
    Posted same day for guaranteed delivery by 1pm next working
    day (if application received by 3.30pm Monday to Friday).
    UK Express: (Saturday delivery): +£14
    Guaranteed delivery by 1pm Saturday (if application received
    between 3.30pm Thursday and 3.30pm Friday).
    UK Express Plus: +£20
    Posted same day for guaranteed delivery by 9am next working
    day (if application received by 3.30pm Monday to Friday).
    UK Express Plus (Saturday delivery): +£24
    Guaranteed delivery by 9am Saturday (if application received
    between 3.30pm Thursday and 3.30pm Friday).
    Overseas Standard: +£5
    We aim to post within four working days by Royal Mail's Tracked
    or Signed accelerated air mail service.
    Overseas Express: +£10
    Posted same day by Royal Mail's Tracked or Signed accelerated
    airmail service (if application received by 3.30pm Monday to
    Friday).
    Overseas Express Courier: +£10 +FedEx fees
    Posted same day using FedEx's International Priority service
    (if application received by 1pm Monday to Friday). Delivery will
    be between 1 and 3 working days depending country (next working.

    Section 6
    Do you require our Legal Copy and Archive Service?
    Organisations such as the Passport Office, DVLA, the financial institutions etc will not accept a photocopy of your Deed Poll document. Our legal copies (also known as certified copies) will be accepted by these organisations in lieu of your original Deed Poll document thus avoiding your original Deed Poll being lost or damaged in the post. Also, we archive an image of your signed Deed Poll so you can order further legal copies in the future should you ever lose or mislay your Deed Poll.

    Section 7
    Do you wish to claim a fee reduction because you have changed your name with us before?
    You can claim a £10 fee reduction on the total value of your order if you have changed your name with us before i.e. if you have previously purchased a Deed Poll direct from us.
    Please note, this reduction cannot be claimed in addition to the fee reduction for receiving a qualifying benefit.

    Do you wish to claim a fee reduction because you receive a qualifying benefit?
    You can claim a £10 fee reduction on the total value of your order if you receive either:

    • Income Support

    • Income related Employment & Support Allowance

    • Pension Credit

    • Income-based Jobseeker's Allowance

    • Housing Benefit

    • Working Tax Credit

    • Child Tax Credit (if your household income is less than £20,000 per annum)

    • Universal Credit (if your household income is less than £20,000 per annum)

    • If someone receives Child Benefit for you

    The fee reduction is only available if you send us written evidence of your benefit entitlement (an original award notice, not fax or photocopy, dated within the past 3 months).     Please note, this reduction cannot be claimed if you are claiming the fee reduction as a previous client.

    If you are paying by debit or credit card and your written evidence does not arrive by the time your application is due to be processed, we will charge the standard fee and enclose a claim form with your Deed Poll documentation, which you can return to us with your written evidence for a £10 refund. If you a paying by PayPal, you will need to pay the full fee but we will enclose a claim form with your Deed Poll documentation, which you can return to us with your written evidence for a £10 refund.

    How will you be paying?

    We will send you an email confirming payment details.

    Complete this section if you are paying by debit or credit card
    If paying by card, we accept Mastercard, Visa, Visa Debit, Maestro and American Express.
    You can use someone else's card if you have their permission but we may call the cardholder to confirm their consent. If you have just provided card details on a previous application form, you do not need to provide the card's details again.
    Your card will not be charged until we prepare your deed poll documentation.








    If paying by cheque or postal order
    We will email confirmation of the fee payable. Please make your cheque or postal order payable to UK Deed Poll Service and write your new name and address on the back. If paying by cheque, we cannot post your Deed Poll documentation until your cheque has cleared, which takes five working days. Post your payment to either address below:

    Using our Freepost address costs you nothing but it takes two to four working days to reach us. If you want your payment to arrive sooner, use the address below and affix a 1st class stamp

    Does this application need to be linked to any other application you will be submitting today (or have already submitted today)?
    This will enable us to ensure you receive the fee reduction for additional Deed Polls ordered at the same time (where the Deed Poll address and delivery address are the same).
