If you enter Yes, we will date your Deed Poll for two working days after we post it (when you should receive it by) or you can enter a specific future date.     *Are you making this application because you need to replace a lost Deed Poll or because you need a Deed Poll as evidence of a past name change?

    If this application is to provide you with a replacement for a lost Deed Poll (or a lost Statutory Declaration), or you are applying because you need a Deed Poll as documentary evidence of a past name change (for example, your name was changed by usage when you were a child), we can modify your Deed Poll to include a declaration that states you have been using your current name (i.e. the new name you have entered in section 1 above) for all purposes since a particular date. This declaration explains to those you show your Deed Poll to that it is not for a new name change.
    If you would like this modification made to your Deed Poll, for which a £4 fee is payable, please select the option Yes and then answer the questions below to tell us when and how your name was originally changed to your current name. If you are unsure whether you need this modification made, please call our Helpline for advice or email us.

    If you require this modification, in section 1 above, please ensure you have entered your old name in the current name boxes and your current name in the new name boxes. Also, the address you enter in section 1 above should be your current address and not where you were living when you previously changed your name

    Some organisations (such as the Passport Office, DVLA and financial institutions) will not accept a photocopy of your Deed Poll. To avoid sending your only original in the post (and risking it being lost or damaged) you can order additional duplicates.

    We will send you multiple original Deed Poll documents, all of which you can sign and witness at the same time. Once signed, they will each serve as valid originals for official use.

    Most of our clients order around 5 copies for convenience — while those notifying multiple organisations often choose more to save time and avoid delays.

    If you lose your signed Deed Poll, there’s no official copy of it kept anywhere. That means you’ll need to apply for a replacement Deed Poll and go through the process again. Our Archive Service prevents this.

    Here’s how it works:

    We will include one extra copy of your deed poll in your pack. Simply sign and witness it as normal and then return it to us using our provided pre-paid envelope. Once received, we will scan and securely store a digital copy of your signed Deed Poll in our archives.

    Now it's in our archives, if you ever lose your Deed Poll, you can request further printed copies from us at any time, even years later, without having to go through the whole process again.

    Most of our clients have chosen to protect their name change this way. It's a small step now that saves a huge headache later.
