To change a child’s name (who is under the age of 16) you simply need to complete and submit the application form below, which has provision for 4 children.

We also suggest you read the relevant sections of our article about changing a child’s name by clicking on this link (opens new window). It is important to understand the meaning of parental responsibility and who needs to consent to a child’s name change.
If you do not have sole parental responsibility for your child, after you have submitted your application form, you need to send to us a letter of consent (by email, fax or post) signed by everyone with parental responsibility for the child (a link to an example of the letter we need will be emailed to you after your application has been reviewed).

If the father named on your child’s birth certificate is not the biological father, please read section 9.
